Investigation of Incident Reporting System in Iranian Hospitals: A National Survey

Background and Aims: Incident reporting is a possible alternative for learning from errors. One of the barriers in this way is a deficit in, common standards for collecting, interpreting, and presenting data. In this research accordance with Iranchr('39')s incident reporting system with minimal information Model for Patient Safety Incident Reporting Systems (MIMPS)of WHO were compared. Methods: This is a cross-sectional and applied study. Using the multistage sampling method 5 provinces were selected randomly. All university hospitals in each province (128) were investigated. Hospital websites were observed for having incident reporting form or no in the first three months of 2019.  Content of all Incident reporting form was investigated completely and item to item. Data was analyzed using statistical descriptive and analytic parameters through SPSS-21. Results: Studied hospitals among main data fields of MIMPS have used the Incident identification category with (mean of 1.4) and Incident type category with (mean of 1.2) more than other categories. The least accordance of hospitals with MIMPS was in the Incident detection (mean of 0.07) and Resulting actions (mean of 0.1) elements.it seems that hospitals do not collect and record this data at all. Analyzing data by each province showed that Yazd province with a mean of 0.78 and SD of 0.25 was significantly(p.v=0.003(. in a better situation about accordance with  MIMPS  of WHO rather than others. Conclusion: It seems Iranchr('39')s hospitals act differently from the WHO model. for reliability and better management of safety data the hospitals must move to standardization. Using MIMPS can be introduced and use as a conjoint model throughout the country. It would be caused better access, optimal management safety data and facilitate learning from events.
